Mission 
the Alpha Pack is a non-profit organization for those affected in Wisconsin by the genetic condition Alpha-1 Antitrypsin Deficiency (A1AD). Our mission is to offer family and individual support and awareness for A1AD through meetings, events, screenings, and news.

  • Anyone may submit a nomination; you do not need to be an Alpha Pack member to submit a nomination.
  • Self Nominations are permitted and encouraged.

How to Nominate?

  • Review the position description for qualifications and eligibility requirements
  • Complete the nomination form.

Nominators do not need to contact the nominee prior to submitting the form. The Alpha Pack will contact eligible nominees for their interest and required documentation.

A person may be nominated for more than one position, however, specific qualifications must be provided for each position.

Documentation Required

Nominee

Nominees must provide:

  • Confirmation of willingness and ability to serve;
  • Biography;
  • Major Accomplishments;
  • Qualifications;
  • Position Statements

Nominations that do not have the required information will not be considered.

Information should be submitted via the nomination form.

Nominator

Nominators must provide:

  • Nominator's name;
  • Nominator's email & phone;
  • Recommended position;
  • Recommendation on the nominee's qualifications;

If nominating a member, you do not need to contact the nominee for their information. The Alpha Pack will contact the nominee for the required documentation.

Nomination Deadline

March 10th, 2010

Confidentiality of Nominations

  • Most nominations are kept confidential.
  • The nominations made public are individuals recommended to the Board.
  • The identity of the nominator is not disclosed to the nominee.

Status of Your Nomination

  • A nomination does not guarantee an appointment to the position.
  • All nominees are contacted after the Board of Directors meeting March 11, 2010. Notifications are sent typically within a week of the meeting.
  • All nominees are advised, before the Election slate is made public, if they have or have not been recommended to the Alpha Pack position. Notifications are typically in September or October.
  • All nominees on the Election slate are notified of the election results immediately following the election. The election typically is in November.
